Frequently Asked Questions​


How do I schedule an appointment?

Please call 508-682-0186 or email us to schedule an appointment.  While we sometimes have last minute openings, we recommend that you contact us at least 2 weeks prior to your desired appointment date.  Our office manager will discuss scheduling, payment options and answer any other questions that you may have about any of our services.  

Can I use my Health Insurance?

We accept the following health insurance plans for Physical Therapy:

  • Blue Cross Blue Shield
  • Harvard Pilgrim
  • Medicare
  • Tufts

If you have a health insurance plan not listed above, and are allowed out-of-network benefits, you may be entitled to reimbursement for Physical Therapy services. For out-of-network cases, payment is required at time of service; as a courtesy we will gladly supply an itemized invoice and the necessary information for you to submit forms to your insurance plan for reimbursement.  We do not accept Worker's Compensation or Motor Vehicle Insurance, but cash-pay options are available for these cases as well.

What about self-pay options?

For services not covered by your health insurance, such as fitness, sports conditioning and wellness services, we offer cash-pay options.  Please contact us for more details.  If you are using your health insurance for payment for Physical Therapy, please be aware that there may be fees that you will be responsible for, including any co-payment or co-insurance as well as any applicable deductible.  We accept cash, check, MasterCard, Visa and Discover for payment of any fees.

Do I need a doctor's prescription or referral?

We do not require a doctor's prescription for any of our Physical Therapy or Wellness services.  However, if you are seeking insurance payment or reimbursement,  a referral or prescription may be required.  Please contact your insurance company to find out if a referral or prescription is required.   Alternatively, you may contact us and we will assist you in determining if a referral or prescription is required.   

How long will my sessions last?

All Physical Therapy and Wellness sessions are scheduled for 30, 45 or 60 minutes, depending on the specific service.  With the exception of small group fitness classes, all sessions are one-on-one with your Physical Therapist.  Each Physical Therapy and Wellness program is unique to the individual, and is based on specific, personalized goals.  The duration and frequency of sessions will be determined with your PT during your initial evaluation.

New Client Info


New patients should attempt to arrive 10 – 15 minutes prior to your first appointment time to complete patient registration forms. To save time, you may download the forms from our website and complete them prior to your appointment. Wear comfortable clothes which will allow ease of movement during exercise such as: sweat pants, shorts, tee shirt and sneakers or rubber soled shoes. We will provide gowns for patients being treated for neck and back conditions.


What to bring on your first appointment:

  • Health insurance ID card
  • Picture ID
  • Prescription from your referring physician, nurse practitioner, physician’s assistant or dentist, if required by your insurance company
  • Referral from PCP if your insurance requires one
  • Please download and print the following forms and bring them with you; if you are unable to access or print the necessary forms please arrive 10-15 minutes early for your appointment in order to complete them at our office
